Every patient referral, whether a friend, family member or a co-worker brings joy to our hearts as it is the best compliment we can receive. As a token of appreciation, for every person you refer to our practice, you will receive a $50 gift card towards any dental treatment.
Simply fill out this form and when your friend, family member, or co-worker schedules their appointment we will send you a $50 gift card in the mail along with a thank you card. Please be sure to hit the submit button at the end of the form, and one of our staff members will reach out to you as soon as possible.
Patient Referral Program Rules:
- A referral is someone who is a new patient in our practice.
- Referred patients must let our staff know who they were referred by.
- One $50 gift card for dental treatment per person referred.
